Vaccine Questionnaire – Used for anyone as long as the person is an adult coming to be vaccinated OR if a child, the guardian is coming with the child to sign the consent in house and give insurance info.
Vaccine Consent for parents – Used most often for school located clinics but also for kids coming in without their parent or legal guardian.
Flu/Covid form – can be used by adults for only flu and/or covid. If used for a child, parent/legal guardian must accompany to give insurance info and sign consent.
Flu/Covid consent with insurance – can be used by adult or parent for child. Has all info and consent portion.
