Community Health Worker
Improve the health of your community – one person at a time! First District Health Unit is taking applications for a Community Health Worker (CHW) to provide support services to qualifying local individuals with chronic diseases, or who are at risk. The CHW can help guide clients through the health care and social service systems, provide coaching and education, and identify and address barriers to health. This position is 15 hours per week at $26.11/hour. The Location is flexible within First District’s seven counties. The Community Health Worker must become certified within 2 years.
