If your employer is offering a flu clinic, please register for the time and date we will be at your worksite. Complete only the required fields on appointment preregistration form. You will receive a confirmation email AND a second email with a link to the online consent form. If not, check your spam or junk folder. Complete the online registration/consent form and sign by typing your name. (When you click SUBMIT, the screen will go blank, but your submission will be received.)
