2023 School Flu Vaccination Clinics

Flu vaccination clinics are only held one time at each school, and all students at that school are registered for the same date and time.  The exact time your student will be vaccinated may be later than the clinic start time.  To register your student:

• • Step One:  Preregistration
    • Find your school’s name in the table below (Quick tip: type school name in the table’s Search box)
    • Note the date and time of the clinic at your school.
    • Click that date and select the time for your school.
    • Complete only the required fields on the appointment system.
    • Click on Complete Appointment.  You will see a confirmation screen and receive a confirmation email.

• • Step Two: Registration
    • You will receive a second email with a link to the online consent form.  If not, check your spam or junk folder.
    • Indicate the consent IS for immunizations at school.  Fill in the school name and grade, and the teacher’s name if applicable.
    • Answer health questions in the first section.
    • NO COVID vaccines are given at school.  Disregard COVID questions section.
    • Complete insurance information.  Be sure to enter policy number and insurance name.  If you have a group number, please fill it in.
    • SIGN CONSENT by checking the box and typing your name.
    • Click submit.  The screen will go blank, but your submission will be received.

Insurance will be billed for the flu vaccine.  If you don’t have insurance, you will be billed. $20.90.  Programs are available to help cover the cost.

Paper registration forms are also available through the school office.
