Flu vaccination clinics are only held one time at each school, and all students at that school are registered for the same date and time.  The exact time your student will be vaccinated may be later than the clinic start time.  To register your student:

Online Registration – Please try to register your child at least two weeks before their school’s clinic date.

Step One:  Preregistration

1. Click here and choose Tuesday, October 10, 2023 • 10:00am
2. Complete only the required fields on the appointment system.
3. Click on Complete Appointment.  You will see a confirmation screen and receive a confirmation email.

Step Two: Registration

1. You will receive a second email with a link to the online consent form.  If not, check your spam or junk folder.
2. Choose immunizations at school option. Fill in the school name, grade and the teacher’s name if applicable.
3. Answer health questions in the first section.
4. NO COVID vaccines are given at school.  Disregard COVID questions section.
5. Complete insurance information.  Include insurance name, policy number and group number (if you have one).
6. SIGN CONSENT by checking the box and typing your name.
7. Click submit.  The screen will go blank, but your submission will be received.

Insurance will be billed for the flu vaccine.  If you don’t have insurance, you will be billed. $20.90.  Programs are available to help cover the cost.

Note: We will not be sending home paper forms. If one is needed, they will be available at the school office for pick up.
