The C & Y/CHNC Program is comprised of a team of professionals providing health screenings for children 6 months through 6 years of age including:- Vision screening
- Hearing and speech assessment
- Development assessment
- Nutrition intake
- Social intake (child's birth history and concerns you may have
- Physical assessment
- Immunizations (don't forget to bring your child's immunization record)
- Referrals
